My MLB headphone amplifier has blown 2 fuses this year; the first time it took out a Sovtek 5Y3 rectifier tube (no great loss) but the second time, the other day, it took out a vintage, speckled brown base CBS 5Y3WGTA rectifier.

What is causing these tubes to arc and blow the fuse? The unit is in an office system with the power cord connected to a strip. The other tubes are good, just tested this weekend after the rectifier blew. The rectifier tested all shorted out.
